When submitting this form electronically, you are acknowledging that as Torch Run Coordinator for your department, you are accepting responsibility for the Torch Run T-shirts and hats order.

You understand that you must receive a $13.00 donation for each T-shirt and $15 donation for each hat. You will be responsible for depositing all funds from the T-shirt and hat donations as soon as possible and will return any extra T-shirts and hats to SONC when requested to do so.
